The following form is required for eligible exchanges of Hill IFC or HF54+ Units that are under a 1 year warranty from date of shipment.

3. Safely package the original product in the new product box and affix the provided FedEx label
4. Schedule FedEx pickup or drop off at a local terminal.

By submitting this form: I agree that the information entered on this form is correct before submission. I agree that upon receipt of a new replacement unit, I am responsible to send the original product identified on this form to Hill Laboratories within 30 days of receipt of the replacement product. I agree that if the original product is not received within 30 days of receiving the replacement product, I will be responsible to pay the full purchase price and shipping fees of the replacement product. I agree I am to package the original product responsibly and affix the package with FedEx Label provided by Hill Laboratories. I agree that if the product is found to have no errors, I will be required to pay $250 service/processing fee.
